A breast lift does not provide the same volume control as an implant. An implant does not reliably correct significant ptosis. Reduction is not simply a smaller implant operation, and fat transfer has different volume limits and biological behaviour.
Examination evaluates breast size and shape, skin quality, nipple–areola position, fold relationship, asymmetry, tissue distribution and chest-wall proportions. Medical history, smoking or nicotine use, medications, pregnancy plans, breast screening and previous surgery can change the safest plan.

The pathway depends on the main concern: adding volume, improving breast position, reducing excess breast tissue, correcting asymmetry, or addressing nipple anatomy. An in-person consultation is needed because more than one concern may be present.
Augmentation mainly adds volume, while a breast lift mainly raises and reshapes a stretched skin envelope and repositions the nipple–areola complex. Some patients may need a combination when both volume loss and significant drooping are present.
Implants can restore or add volume, but they do not reliably correct significant breast ptosis. When the nipple position and skin envelope need correction, a breast lift may be considered with or without augmentation.
